<blockquote><p>The submitted picture represents an incision 2 days after a vasectomy procedure with a single midline incision.  There is minimal bruising and swelling surrounding the wound.  The dark areas represent some skin that has been strangulated by the sutures used to close the incision.  There appear to be 4-5 temporary sutures.  Everything appears to be healing as expected for a man after a vasectomy performed with a single midline incision two days after the procedure.  The single midline incision sounds attractive to prospective patients, but the reality is that a single incision that requires 4-5 sutures to close is much more invasive than two separate incisions requiring zero to one suture to close.  A comparison of photos from previous posts using the 2 incision technique will help with understanding the differences between the two approaches.</p></blockquote>

<p>I would like to ask a question if I may:- Is the amount of bruising something I should be worried about?</em></p>
<blockquote><p>These pictures represent some greater than expected bruising after vasectomy.  The bruising extends to the base of the penis and across both hemiscrotal areas.  There seems to be minimal swelling which would suggest that the bruising is due to some bleeding in the skin layer and not a hematoma in the scrotum.  The second picture demonstrates one of the incision sites that appears to be healing appropriately.</p></blockquote>

I&#8217;m pretty sure this is considered &#8220;normal&#8221; healing because the incision was swollen like this since I first peeked under the bandage. I have had minimal scrotal swelling (says my fiancÃ© &#8211; I honestly don&#8217;t notice it) and no bruising. It&#8217;s just the incision that didn&#8217;t seem to match up with any photos I&#8217;ve seen here. Would you classify this as normal? Is this maybe just another method of stitching my doctor preferred (like the different methods of healing a newborn&#8217;s belly button)?</em></p>
<blockquote><p>The midline incision for performing vasectomy is one approach to this surgery. A small amount of swelling can develop at the incision and beneath the suture used to close the incision. The incision does demonstrate a small area of scrotum that is congested under the suture line. <p>2 1/2 days ago I had a hernia repaired &amp; a vasectomy at the same time. During the pre operation consultation, the doctor said that it would be easy to cut the vas using the same incision as the hernia repair. He said that he would have to make a small incision on the left side of my scrotum to cut the left side. That all sounded great &amp; easy.</p>
<p>Well I guess that my question is: Is there reason that there should be a 1/2&#8243; cut on the top side of my penis near the base? I have never heard of a doctor getting to the vas deferens through the top of the penis. This is a little alarming to me because nothing was mentioned about an accidental cut during the process when I woke up. It seems that the cut had been &#8220;glued &#8221; together because I see no sign if a stitch. My wife says that there is a couple of small stitches on the underside of my scrotum, but with the swelling I am unable to see them for myself. I feel that there is an additional cut where there shouldn&#8217;t be. Have you heard of cutting the top side if the penis to access the vas deferens?</p>
<p>I have attached 2 pictures one of the hernia incision, which it looks like he used a previous scar from an earlier surgery. You can see swelling below the incision. The second picture is of the cut mentioned in the question above.</em></p>

<blockquote><p>Sometimes, surgeons will offer to perform a vasectomy through the same incision as the hernia repair. This is a convenient approach for vasectomy. However, the patient should be informed that performing a vasectomy reversal is significantly more difficult in the groin than in the scrotum as it is normally performed and this might limit a patients options in the future. I have never heard of a vasectomy being performed through a penile incision like the one pictured in this case. Since there is an incision over the left hemi-scrotum and the vas deferens is easily accessible through the right hernia incision, I can&#8217;t explain what the additional incision was used for. Perhaps asking the surgeon who performed the surgery the reason for this incision and posting a follow-up message would be useful.</p></blockquote>

			<content:encoded><![CDATA[<p><em>Thank you for taking the time to look at my question. Today is Wednesday. Monday at 3:30 PM I had vasectomy. I am still experiencing a high level of discomfort which I think was expected but my concern is the location and specific pattern of my bruising. The bruising seems to follow straight up the path of my urethra. I just would like to know if there is cause for concern. </em><em>I have attached a <a href="http://www.vasectomy-information.com/pictures/bruisingonpenis.htm">photo of the bruising on the penis</a>. The photo was taken a few minutes ago, and thus, is the most recent.</em></p>
<blockquote><p>Bruising after a vasectomy can extend in any direction from the vasectomy incision site. In this case we see the bruising tracking up the penis along the course of the urethra. Urethral damage has been reported at the time of vasectomy in a few rare instances and this is more likely to happen with a midline incision as compared to bilateral incisions. A urine analysis should be performed to rule out microscopic blood in the urine if no gross blood is seen when urinating. Any blood in the urine should be further evaluated with specialized x-ray tests of the urethra and possibly urethroscopy (endoscopic visualization of the urethra). Blood in the urine would suggest a urethral injury. The appearance of bruising in this picture suggests damage to at least the corpus spongiosum, the vascular structure surrounding the urethra. This can occur without damage to the underlying urethral lumen.</p></blockquote>

On January 16, my husband underwent a bilateral vasovasostomy, which, according to the urologist, went extremely well. Unfortunately, that was not the case. My husbands scrotum, IMMEDIATELY following surgery, started swelling, and turning dark purple, then nearly black. Please look at the <a href="http://www.vasectomy-information.com/pictures/vasovasostomyhematoma.htm">photo&#8217;s</a> I took of this. I started documenting this information because I was extremely worried that these symptoms would worsen. And I was right.</p>
<p>Within 8 hours after the procedure, my husband started experiencing tremendous pain accompanied with bruising and swelling in the right side of his groin. We prayed that this would be normal side effects from the reversal, and would eventually ease.<br />
Today is the 26th of January (nearly 2 weeks after the procedure) and there has been little improvement and much more pain.<br />
On January 23, I took him to the emergency room where they diagnosed him with a scrotal hematoma, prescribed him ten &#8211; 5mg percocets and sent us on our way.</p>
<p>Since that night my husband has had a low grade fever that goes up and then down within minutes. The fever usually lasts about 5-10 minutes and is associated with sweating. The fever spells seem to have subsided within the last 12 hours, but he has not been able to get out of bed AT ALL for nearly 3 days, as the pain is excruciating in his scrotum when he stands up, and he SAIS that it feels as though his testicles are going to burst (at that point his testicles are swelling again).</p>
<p>The pain in his groin is subsiding, but like I mentioned, his testicles are in severe pain as soon as he stands up. He nearly passes out and sweats and shakes from the pain (I fear that he might go into shock). Since the surgery, my husband has been bedridden &#8211; and I have devoted my time researching any possible articles that may relate to this situation, but have been unsuccessful in doing so. The only scrotal hematoma that I have been able to find are ones from vasectomy&#8217;s, not vasectomy reversals.</p>
<p>My questions to you are:<br />
1.) Is this extremely dangerous, and what do I need to do? I know I should take him to the emergency room, but they will probably do the same thing that they did the last time, as this is a military hospital, and his surgery was done at a Medical Center &#8211; several hours from where we live.</p>
<p>2.) Could this hematoma harm the vas deferens since the hematoma may have been a result of bleeding from the vas deferen?</p>
<p>3.) Also, what are his chances for fertility when a scrotal hematoma occurs?</p>

<blockquote><p>Scrotal hematoma can be a complication of ANY scrotal surgery and is not just limited to vasectomy procedures.  In fact, the more involved the scrotal surgery, the higher the risk of developing a scrotal hematoma.  Meticulous care to control all possible bleeding sites at the time of surgery is necessary to prevent scrotal hematomas.  Blood from a scrotal hematoma can track up the abdominal wall or down into the perineum.  Blood tracking into the perineum will cause a characteristic butterfly pattern as seen in these pictures.  After two weeks, there is nothing surgical that can be done to minimize the bruising.  In fact, bruising at this point represents old blood that has already begun to degrade and will go through a spectrum of color changes like any other bruise.  Since the active bleeding most likely has stopped, the best course of action would be to wait for the hematoma and bruising to resolve which can take several months.  It is difficult to say if any harm to the vasectomy reversal will come from this hematoma.  The hematoma will inevitably cause some scarring in the scrotum.  The extent of the scarring and the damage due to the scarring is unpredictable.  The best indicator of the success of the surgery will be based on the results of the semen analysis.</p></blockquote>

<p>Firstly, one of Dr. Karpman&#8217;s patients has kindly consented to us posting pictures of his vasectomy incisions taken just after the procedure.</p>
<p>Secondly, we have pictures of a midline incision vasectomy taken on day 1 just after the vasectomy, and the subsequent healing and bruising on day 4.</p>
<p>Lastly, we have pictures of vasectomy scars taken some years after vasectomy. One set is a bilateral vasectomy with photo&#8217;s taken 10 years afterwards, the other is a midline incision vasectomy with photo&#8217;s 7 years afterwards.</p>
